Welcome to Putnam Community Medical Center Registration Department. The Registration Department provides a customer focused environment and we are committed to providing you with the best available healthcare along with a convenient and reliable registration process.
Upon your arrival for outpatient services or inpatient stay, you will be greeted by a registration representative who will verify your current information. You will be requested to sign a "Consent for Treatment" and "Consent to Bill" form. You may also be asked to sign other forms that may be required by Medicare or other regulatory agencies. All patients will receive the "Patients Bill of Rights."
Please have the following items ready when you arrive this will insure that the registration process will flow smoothly and efficiently:
- Current health insurance information
- Picture ID
- Physician orders
- Living will for Surgery/Inpatient Care
Please contact us if you have questions, changes in your insurance coverage or request to be Pre-registered to expedite your process on the day of your procedure, or just to update your existing information. We are available to assist you during the following times.
Outpatient/Pre Admission Registration
Monday through Friday: 7:30am - 4:30pm
Emergency Room Department
7 days per week 24 hours/day
Monday through Friday 8:00am-4:30pm
Patients are responsible for all financial liabilities including co-payments, deductibles and out of pocket expenses at the time of registration or before being discharge from the hospital. If the exact dollar amount has not been determined, you will be billed an estimated amount. Please contact the hospital's financial counselor at (386) 326-8214 if you are unable to make payment before registration or prior to discharge.
